Job summary
We are looking for a Health Care Assistant to work as part of the Responsive Services team in First Community Health and Care. Responsive Services include the Virtual Ward, Intermediate Care Team, Referral Hub, and Urgent Community Response team.
The Health Care Assistant will work collaboratively with the multidisciplinary team including Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Health Care Assistants, Rehabilitation Assistants, and Administrative Staff. The post of Health Care Assistant involves supporting patients to carry out and progress treatment programmes in patients' own homes as directed by qualified staff.
This is a fixed-term job for 12 months to work on a project to support patients who have fallen in their homes. This will involve supporting them off the floor with appropriate equipment, carrying out safety checks, and liaising with clinicians to support the patient and arrange the required assessments and follow-up.
Due to the nature of the role, it is essential that applicants are able to drive and have access to a car suitable for business use. The use of public transport to complete this role is not possible due to the area that we cover.
The service operates from 08:00-20:00, 7 days a week, 365 days a year; the post holder will be expected to work on a rota basis.

Central to this role are:
* An interest in working in rehabilitation of patients in the home setting to improve patients' independence and achieve their goals
* Experience of working autonomously and as part of a team
* Must be able to manage time effectively
* You must be a car driver and have access to a car

* Working both unsupervised and supervised, reporting patient progress and escalating appropriately to senior clinician
