Job Title: Adult Social Worker – Disabilities Team

An exciting job role has recently become available at Sandwell. They are looking for a new member for their Disabilities Team.
What will your responsibilities be?
In this role, you will conduct assessments of service users’ needs, strengths, and weaknesses, and develop tailored intervention plans with measurable goals. You will monitor and evaluate clients’ progress, adjusting treatment plans as necessary. Providing crisis intervention and empathetic support to clients is also a key responsibility. You will collaborate with multi-disciplinary teams, including Occupational Therapy, Physiotherapy, and Intermediate Care Support Workers, to work towards independent living outcomes for service users. Additionally, you will maintain accurate records and documentation.

Qualifications and Experiences :
You must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree level or equivalent qualification in Social Work are also required.
