Job summary This role presents an exciting opportunity to contribute to the development of the specialist community midwifery team for vulnerable women (Indigo Team). With a focus on incorporating principles of social prescribing in line with the Pregnancy Anticipatory Care model the post holder will work alongside our community midwifery teams to proactively identify and support families at risk of poorer outcomes. We believe that utilising specialist, trained assistant practitioners (maternity) will allow integration of services around the family, improve choice and control and give time which focuses on 'what matters to them'. A holistic approachto health and wellbeing will include connecting families to appropriate communitygroups and services. Main duties of the job In this role you will be responsible for; Providing vulnerable families, with a focus on teenage parents and those seeking asylum, tailored support to enhance their care and improve outcomes Working within the specialist community midwifery team (Indigo Team) to develop personalised care plans Working as part of the multi-agency team around the family including through safeguarding procedures Signposting to community and specialist services and working within the wider multidisciplinary agencies, initiating links within the Family Hub services, community and VSCE organisations Managing and prioritising your caseload and working flexiblyin the community setting Advocating for women both within maternity services and the wider community including by identifying and addressing barriers to engagement and supporting them toincrease their ' health literacy ' Promoting physical wellbeing, such as providing aqua natal classes and walking groups Co-delivering Antenatal Programmes relevant to each local authority health visiting team Delivering essential baby items and equipment sourced via local baby banks Supporting the learning needs of student midwives Data collection and evaluation of the role throughout its implementation Utilising theNational Academy of Social Prescribing (NASP)Portfolio of Evidence as a tool to evidence and reflect on personal practice and identifytraining needs and opportunities Driving and access to own car is essential for this role About us Leadership and Improvement Training South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff.
