The post holder will provide expert care to children who are living in foster care with their carers or families.
Main duties of the job
Looked After Children
1. To provide high quality holistic assessments and monitoring of the health status of children and young people in accordance with national and local standards.
2. To undertake health assessments and produce health summaries and health care plans that identify the health needs of children and young people and any action that needs to take place during the following year. The reports need to be understood by social workers, birth parents, foster carers, young person, and other health professionals.
3. To assist care leavers to gain independence and the skills to access mainstream health services and provide a health care plan on discharge from the service.
4. To undertake one to one health promotion sessions on identified health needs with young people or their carers, on topics such as sexual health, drugs and alcohol, fitness, emotional health, and other relevant topics in partnership with experts who may support the service in delivering on some subject areas.
Child Protection
1. Liaison with the multi-agency network and the wider medical team.
2. Provide clinical support to Medics on a rota for Child Protection medical assessments.
3. To assist children, young people, and their families during child protection medicals including attending skeletal surveys, blood taking, CT, and intimate examinations if indicated.
4. Ensures effective collaboration and communication with multidisciplinary team, patients, carers, family, and significant others whilst always seeking consent of the client.

Person Specification
Qualifications
* BSc Nursing or RN currently registered with NMC.
* Registered Specialist Community Public Health Nurse (HV/PHN).
* Practice assessor/supervisor.
Experience
* Post registration experience of working with children and young people aged 0-18 years within the community setting.
* Knowledge of key legislation and policy relating to looked after Children.
* Experience of working with children and families with safeguarding concerns.
* Experienced in developing care plans.
* Experience of Multi-Disciplinary Team working.
* Experience of teaching, training, and/or supervision.
* Evidence of recent CPD.
* Proven ability to write detailed reports and communicate complex information to multiple agencies including Social Care, voluntary sector, primary, secondary, and tertiary services.
* Evidence of working autonomously, using professional judgement to clinically assess children & young people with complex health needs.
* Knowledge of relevant legislation and agendas relating to Children Looked After, Safeguarding, and Child Protection.
* Evidence of understanding patterns of abuse and neglect.
* Evidence-based knowledge of child development (0-18).
* Experience of working with vulnerable children and adolescents who may be on the edge of care, in care, or leaving care.
* Experience of delivering clinical care to children and young people who may have complex health needs.
* Experience in developing and delivering programmes of health education and health promotion.
* Knowledge and understanding of child sexual exploitation and associated risks.
