We are seeking experienced and dedicated Social Workers to join our Adult Social Work Teams in Wrexham, specifically within the Single Point of Access (SPOA) team and the Team for Older People. Key Responsibilities:
1. Conduct thorough assessments of service users' needs and provide appropriate interventions.
2. Develop, implement, and review care plans in line with legislative requirements and best practices.
3. Work collaboratively with other teams, services, agencies, and organisations to ensure a coordinated approach to care.
4. Prepare and present reports to a high standard for various audiences, including multi-agency meetings and legal proceedings.
5. Prioritise and manage workloads efficiently to meet service demands.
6. Apply a strong understanding of social services role, function, and organisation in decision-making and service delivery.
7. Uphold the values and behaviours outlined in the Code of Professional Practice and Council values.
8. Work independently and use initiative to provide high-quality support and interventions.
Essential Skills & Experience:
1. Well-developed communication skills, both written and verbal.
2. Proven ability to write and present high-standard reports.
3. Strong assessment skills, ensuring appropriate and effective interventions.
4. Excellent organisational skills with the ability to prioritise competing demands.
5. Strong relationship-building skills with internal teams and external agencies.
6. Ability to work autonomously while also contributing effectively to a team environment.
