Job summary Band 6 Team Leader- 37.5 hours Our Complex Care Team is made up of: The Children's Continuing Care Team, which provides respite in the home and school setting for children with complex needs. The Long Term Ventilation Team which provides packages of care in the home for children who require ventilation to assist their breathing. Swanbourne Gardens which is a 4 bedded respite unit that provides overnight respite for children with complex needs. As a team we strive to provide the children we support with a high standard of care. Main duties of the job As a Band 6 Team Leader working within the team you would work across the 3 teams based out of Swanbourne Gardens - our unique 4 bedded respite unit. Alongside this you will be required to assess, plan, implement and evaluate patient care in the child and families own home or in school / nursery. Communication is an integral component of this role and communicating effectively and regularly with our teams and the parents and families is vital to ensure the effective management of our services and the provision of appropriate levels of support. You will be at the forefront of teaching and assessing the competencies required for the staff to deliver day to day care for the children with complex health needs, this will include interventions such as care and management of tracheostomies, gastrostomies and ventilation support. You will be expected to participate in the active management of the rota to ensure delivery of commissioned packages of care and the provision of supervision and appraisals for the all of teams and ensure all mandatory training is up to date.
