Are you an experienced physiotherapist wanting to try new experiences and challenges within the Mid and North Hampshire area? Are you passionate about delivering patient centred care with a focus on improving quality of life whilst providing rehabilitation in the patients' own environment? The teams are responsive, well led, and caring, providing guidance to patients, relatives, carers, and staff.
Our innovative Enhanced Care at Home Team and the Mid Community Therapy Team are looking for a physiotherapist for a year maternity to provide holistic assessments and treatments to the population of Mid and North Hampshire. The role will cover the nursing home patients within the division and the community patients for the Winchester patch, working alongside occupational therapists, therapy assistants, and carers to improve the quality of life of the service users.
Main duties of the job
1. The Community Therapy Team model aspires to provide a responsive, safe effective caring service that is well led. The team is commissioned to reflect the needs of the local community. The service aims include providing a local point of access, proactive case management, and adoption of a co-produced approach with patients and service users.
2. To assess and treat own caseload, which may include complex presentations and to maintain own records as an autonomous practitioner.
3. To assist senior staff in the education and supervision of junior colleagues, assistants, carers, and students.
4. To assist senior staff in daily management of the team and provision of therapy service, deputising for the team leader in their absence.
5. To assist senior staff in development and maintenance of an efficient and effective service.
6. To contribute to the inter-disciplinary working within the therapy team, nursing home care team, and Locality by teaching, learning & practicing inter-disciplinary skills & techniques as defined by an agreed competency framework or training scheme.
7. To provide specialist therapy skills to the therapy team across a five/seven-day rota.
8. To provide specialist therapy skills to the Nursing Homes within the Mid and North Hampshire area.
