Are you an experienced adult social worker looking for a new opportunity? Westminster Hospital Discharge team are in need of a new social worker to replace someone who is going on maternity leave. They will offer a competitive rate and hybrid working.
What will your responsibilities be?
As a Social Worker, you will be undertaking section 2s and 5s, facilitating hospital discharge, carrying out assessments in different wards, managing a busy caseload within an acute hospital setting, working closely with health colleagues, ensuring adults in hospital are discharged in a timely and appropriate manner, attending multi-disciplinary meetings, conducting mental capacity assessments leading to best interest decisions, and advising patients and families who do not meet the eligibility criteria and redirecting them to other agencies.
