An exciting opportunity has arisen within the South Community Mental Health Team for a Community Psychiatric Nurse / Social Worker or OT to provide integrated health and social care to people of working age who have moderate to severe mental health problems across a wide range of presenting conditions. The post will be based with the North Community Mental Health Team.

Key Responsibilities of the Post:
As a Mental Health Practitioner, you will be responsible for the healthcare of patients under secondary care.
Main Duties of the Job
* Act as Care Co-ordinator and carry a caseload
* Assess needs
* Implement care plans
* Assess and monitor risk
* Monitor, review, and evaluate care
* Liaise with all agencies and members of the multi-disciplinary team
* Carry out clinic duties such as Clozaril administration or depot injections
* Supervise Community Support Workers / Band 5s and peers as appropriate
* Maintain accurate, up-to-date electronic records
* Conduct initial mental health and social care assessments
* Participate in the CMHT’s duty system on a Rota basis
* Work in a therapeutically informed way within the multi-disciplinary team
* Maintain a persistent approach to engagement with complex mental health difficulties
* Prioritize and work flexibly with MDT
* Promote physical health and wellbeing
* Train as a Safeguarding Adults Manager if not already a SAM
* Complete funding applications and review placements and care of service users
Person Specification
Education and Qualifications
* Registered Mental Health Nurse / Occupational Therapist / Social Worker
* Current registration with appropriate regulatory body (e.g. NMC, AHPC)
* Completion of appropriate post qualification consolidation (e.g. Preceptorship or ASYE)
* Qualification in psychological therapeutic interventions
Experience
* Substantial experience of relevant post-registration practice
* Experience of managing a caseload
* Experience of working in a multi-disciplinary team setting
* Experience of working with people with severe and complex mental health problems
* Experience of clinical supervision of other staff
* Experience of working within a Community setting
Knowledge
* Good working knowledge of therapeutic assessment techniques
* Knowledge of screening and assessment tools for severe and complex mental health problems
* Understanding of Safeguarding adults procedures
* Understanding of the Mental Capacity Act, Mental Health Act, and Care Act
* Understanding of roles under the Care Programme Approach
* Knowledge of key national and local policies in adult mental health services
* Knowledge of local authority and CCG commissioning processes
Skills and Abilities
* Ability to undertake the care coordinator role
* Excellent written and interpersonal communication skills
* Ability to manage own emotions and deal with stressful situations
* Ability to engage with and provide therapeutic interventions
* Ability to prepare and complete relevant care records
* Ability to manage a diverse and complex caseload
* IT Skills
