Hutton View is an elegant award winning 77 bedded Residential, Nursing and Dementia care home offering Relationship-Centred Care, supporting residents at the heart of all we do. Then read on About the role: Hallmark Care Homes’ dementia communities operate as a home from home environment. As a Dementia Care Manager, your prowess and magical touch will lead and manage a committed and loyal caring team on the community and be responsible for driving all aspects of the Hallmark dementia strategy, ensuring your community meets the stringent standards we have specially created for excellent relationship-centred dementia care. You’ll oversee the Care Plans for all Dementia residents in the home, ensuring the Care and Nursing teams are aligned and engaged in providing an individualised plan for every resident. You’ll work with the CQC, ensuring relevant documentation and relationships are maintained in line with regulatory requirements. You’ll be responsible for ensuring the team providing Dementia support are fully trained and compliant, providing training to the team personally and directing them to changes in policies and practices. You’ll be working with other team members in the Home, i.e. Hospitality Manager, Head Chef, Cleaners etc, to ensure they are aware of any individual needs. You’ll be communicating regularly with families, GP’s and other professionals. You’ll build relationships with other Dementia Care Managers across the other 22 Hallmark Luxury Care Homes, identifying common successes and challenges to ensure a consistent way of working. You’ll be the ‘go to’ person for Dementia within the Home and be the expert with the knowledge and experience to advise on best practice. What we need from you - You have supervisory or management experience, (this role would suit a Senior Care Team Lead / Unit Manager or Nurse from with a Strong Dementia background, who is looking to take that next step in their career). - You have experience and knowledge of evidence-based practice in dementia care, including in long term care of the older person, including people living with dementia. - You’ve already achieved or are willing to work towards qualifications NVQ 4 or 5 / QCF 5 in health and social care. - You’ve got detailed theoretical and practical knowledge of care plans and advanced care plans in respect of end-of-life care. - Ideally, you’ll have some experience in training delivery. - You have experience of working with the CQC and aligning to their processes and expectations. - You have a passion for excellent care and will ensure our Residents have the very best of everything at Hallmark. - You have a true desire to work as a team, engaging and supportive – our homes thrive on collaboration What we offer: Your dedication and hard work will be celebrated with a competitive salary, outstanding training and development opportunities, and a supportive work environment.
