If so, West Sussex CAMHS may have just the opportunity for you Coastal and North West Sussex Children Adolescent Mental Health Service (CAMHS) have recently received funding to develop and expand the fantastic services we offer to young people in our community. We work with a range of children and young people who present with a multitude of mental health problems. We are looking for a skilled mental health practitioner who will take the lead in working with our adult colleagues to identify those young people who will require ongoing care in post 18 services and to support their transitions. We have an identified lead for transitions and an integrative policy framework to guide our practice. The post will suit a practitioner from either child or adult services who has a commitment to ensuring the achievement of good quality transition plans. Main duties of the job Working across West Sussex and in collaboration with CAMHS and AMHS you will play a key role in ensuring our young people will receive a timely and smooth transition from their CAMHS team to their local Adult Mental Health Team. As part of this role you will be involved in working with the family and the MDT to establish the right level of future care needs for each young person. This could involve working with our colleagues in the ADMHS or Tier 2 level services such as Sussex Talking Therapies, Pathfinder and signposting to other local support services.
