To report information to trained staff on progress of patients and any variance/change in patient/carers care management in a timely manner. To complete patient records in line with local record keeping standards and protocols To use IT systems for relevant tasks e.g. accessing patient records and results, completing record keeping. To maintain a patient orientated approach towards care delivery, promoting a rehabilitative and self-care approach To put person centred values into practice and to promote dignity in your day to day work with the individuals you support To follow the Trust guidance in moving and handling and comply with the patient's care plan. To report any issues that may put health, safety and security at risk following the Trust Risk Assessment process. To report accidents/ incidents of patients or self on the Trust reporting system (datix) To report any actual or potential abuse or concerns to the Team Lead and to summon help immediately in a crisis situation. To report to manager/trained nurse any untoward incidences (e.g. complaints, accidents, defective equipment) and take appropriate action to ensure safety of patients, staff and relatives.
