Job Title: Social Work Assistant
Pay Rate: £17.12 per hour
Contract: 3 months (with potential to extend or become permanent)
Hours: Full-time, 37 hours per week (Monday to Friday, 9:00 AM - 5:00 PM)
Location: Bromsgrove, B61

We're seeking a Social Work Assistant to join a dynamic team committed to delivering person-centred support that makes a real impact.
About the Role:
As a Social Work Assistant you'll:
Conduct assessments using national eligibility criteria and create person-centred support plans.
Provide advice, practical support, and assistance to help service users regain independence.
Work collaboratively with individuals, families, and external agencies to achieve positive outcomes.
Facilitate access to services, resources, and universal support systems.
Maintain accurate records and liaise with other professionals to deliver effective care.
What We're Looking For:
Experience: Significant experience in health or social care, particularly with individuals with learning disabilities, long-term conditions, or dementia.
Full UK Driving Licence with unrestricted use of your own vehicle
Qualifications: NVQ/Diploma Level 3 in Health and Social Care (or equivalent).
Knowledge: A solid understanding of the Care Act 2014, Mental Capacity Act 2005, and person-centred support planning.
