Wirral's Adults Community Team South is seeking a dedicated Qualified Social Worker to support the delivery of health and social care functions for adults and carers in the community. This role plays a key part in implementing Personalisation and Inclusion Pathways, ensuring the Care Act 2014 framework, strength-based practice, and social inclusion principles are effectively applied.
Key Responsibilities:
1. Conduct assessments and access appropriate social care services for adults with acute or complex long-term physical and mental health needs.
2. Support and empower older people and carers with advice, guidance, and care planning.
3. Promote strength-based and asset-based approaches in service delivery.
4. Provide supervision and professional guidance to newly qualified social workers and Care Navigators.
5. Work in partnership with health professionals, voluntary organisations, and other agencies to ensure person-centred support.
6. Maintain accurate records and ensure compliance with statutory duties under the Care Act 2014.
Requirements:
1. Qualified Social Worker with Social Work England registration.
2. Experience working with adults with physical or mental health needs, older people, and carers.
3. Strong knowledge of the Care Act 2014, personalisation, and social inclusion.
4. Ability to work within a hybrid model, including community visits as required.
5. Experience in supervising and supporting newly qualified social workers (desirable).
