Health and Wellbeing Coach - Rural PCN (Wincanton Health Centre)
Symphony Healthcare Services have an exciting opportunity to recruit a Health and Wellbeing Coach in the Rural Primary Care Network.
You will be employed by the member Practices of the Rural Practice Network which include Millbrook Surgery, Queen Camel Medical Centre, Wincanton Health Centre, Bruton Surgery and Milborne Port Surgery. For this appointment your lead Practice is Wincanton Health Centre (Symphony Healthcare Services) who will process your monthly payroll and will be acting as an agent on behalf of the Rural Practice Network.
The post available will be up to 37.5 hours per week based at Wincanton Health Centre with working patterns to be discussed at interview. The rate of pay for this position is £11.70-£12.50 per hour, depending on experience with the option of an NHS or NEST pension.
Main duties of the job
A Health Coach will provide support to patients to take proactive steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support people to develop their knowledge, skills and confidence or to build their patient activation - in managing their health and care, to improve their health outcomes and quality of life.
The successful candidate will have good communication, negotiation and people management skills and act with compassion and integrity. The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied and sometimes challenging environment is essential.
Job responsibilities
Key Responsibilities
1. Provide one-to-one and group health coaching support for people with one or more long-term conditions, with the aim of improving people’s levels of ‘activation’, empowering people to manage their own health and improve their health outcomes.
2. Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload.
3. Work as part of a multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self-Management, modelling the coaching approach in their work.
4. Ensure that GPs, practice nurses, practice pharmacists and other members of the primary care team understand the health coach role, how to refer to them, and which patients may particularly benefit from health coaching.
5. Support local health, social care and voluntary sector professionals to make appropriate referrals to the service.
6. Promote and raise awareness of the health coaching service particularly to groups and communities that experience barriers to access.
7. Attend and contribute to team, practice, and PCN meetings and events as required by the service.
8. Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.
9. Participate in regular health coaching supervision and continual learning.
10. Collect service user experience and impact of health coaching as part of the delivery of personalised care.
11. Evaluate people’s levels of knowledge, skills and confidence (their ‘Activation level’) and measure the impact of the coaching provided.
Person Specification
Knowledge and Skills
1. Understanding how to apply health coaching in group settings.
2. Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people.
3. Demonstrable skills in supporting behaviour change.
4. Skilled in active and reflective listening, building trust and rapport quickly.
5. Ability to work with minimal supervision and act decisively.
6. Proficient in MS Office and web-based services.
7. Ability to travel across multiple sites.
8. Ability to work flexible hours and travel out of area as necessary.
9. A good understanding of the evidence base and development of self-management in the UK.
10. Excellent communication and presentation skills.
Experience
1. Experience of using coaching approaches/frameworks and models or other helping strategies.
2. An understanding of the biopsychosocial model of health and the social determinants of health.
3. Experience of working within a team.
4. Experience of working in the NHS or similar health care setting.
5. Networking experience.
Qualifications
1. English and Maths GCSE or equivalent (Grade A*-D / Grade 9-3).
2. Coaching/counselling qualification or another relevant qualification/experience.
