An exciting job role has recently become available at Reigate. They are looking for a new member for Assessments.
Duties:
* Needs Assessments: Evaluate the care and support needs of individuals to determine their eligibility for services under the Care Act 201412.
* Risk Assessments: Identify any risks to the individual’s wellbeing and safety, and develop plans to mitigate these risks
* Care and Support Planning: Create detailed care plans that outline the support and services required to meet the individual’s needs
* Reviewing Plans: Regularly review and update care plans to ensure they remain relevant and effective
* Guidance: Offer information and advice to individuals and their families about available services and support options
* Counseling: Provide emotional support and counseling to help individuals cope with their circumstances
* Crisis Intervention: Step in during crises to provide immediate support and ensure the safety of vulnerable individuals
* Safeguarding: Work to protect individuals from abuse, neglect, and exploitation
* Multi-Agency Work: Coordinate with other professionals and agencies, such as healthcare providers, to ensure a holistic approach to care
* Referrals: Make referrals to other services and agencies as needed to provide comprehensive support
* Documentation: Keep detailed records of assessments, care plans, and any interventions or support providedeporting: Write reports and maintain accurate documentation to support decision-making and ensure accountability

What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
