Abbey Medical Group is looking to recruit an experienced GP Practice Nurse to help support our team on a part-time permanent basis. Experience in managing Diabetes is desirable.
Abbey Medical Group is a diverse training practice with 12,500 patients, over two sites based in Ravenshead and Blidworth, Nottinghamshire.
The successful candidates will contribute significantly to the nursing team rota for both the Extended Access services and our core opening hours.
Salary will be discussed at the final interview stage and will be based on previous experience.

Main duties of the job
This forward-thinking GP surgery is offering an experienced Practice Nurse the chance to join their dedicated clinicians, who provide nothing but the best service for all patients. You will be part of the nursing team running a range of different clinics and be able to focus on your specialist interest. If you have experience in long-term conditions, especially in managing Respiratory, then this could be the post for you. In addition to the standard Practice Nurse qualifications (including Travel/Child immunisations and Cervical Screening), you must also be NMC Registered and ideally have a minimum of two years experience within a GP setting.
The post holder will use their own judgement, resourcefulness, and common sense and show flexibility with regard to rotas and working patterns.

Job responsibilities
WE ARE ONLY CONSIDERING EXPERIENCED PRACTICE NURSES.
The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
Primary responsibilities
1. To develop, implement and embed health promotion and well-being programmes.
2. To implement and evaluate individual treatment plans for chronic disease patients.
3. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
4. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE, and the NSF.
5. To provide wound care (ulcer/Doppler etc.) to patients.
6. To request pathology services as necessary.
7. To process pathology results as required.
8. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
9. To maintain accurate clinical records in conjunction with extant legislation.
10. To ensure read codes are used effectively.
11. To maintain chronic disease registers.
12. To develop, implement and embed well woman clinics.
13. To develop, implement and embed well man clinics.
14. To chaperone patients where necessary.
15. To prioritise health issues and intervene appropriately.
16. To support the team in dealing with clinical emergencies.
17. To recognise, assess and refer patients presenting with mental health needs.
18. To implement vaccination programmes for adults and children.
19. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
20. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
21. To support the clinical team with all safeguarding matters in accordance with local and national policies.
22. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
23. To deliver opportunistic health promotion where appropriate.
Secondary responsibilities
In addition to the primary responsibilities, the practice nurse may be requested to:
1. Support junior members of the nursing team, providing guidance when necessary.
2. Participate in local initiatives to enhance service delivery and patient care.
3. Support and participate in shared learning within the practice.
4. Continually review clinical practices, responding to national policies and initiatives where appropriate.
5. Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA).
