The Team provides a traditional old-school approach to Assertive Outreach based on the model developed from the Mental Health Policy Implementation Guide published in 2001, providing Psychosocial Interventions to Service Users in their own homes.
Main Duties of the Job
You will provide clinical and managerial leadership to the Assertive Outreach Team, holding your own small but complex caseload and providing live supervision, guidance, and support for the team's clinical staff while providing psychosocial interventions to service users. You will be expected to lead and support the implementation of any needed recommendations from the CQC Special review of Notts mental health trust.
You will need to be kind, compassionate, and organized; a sense of humor and resilience is a must as you will form strong relationships with Service Users who can be challenging but are ultimately rewarding to work with. You will represent the Assertive Outreach Team at Local, Regional, and National meetings and be an advocate for the team and for the service model.

Job Responsibilities
JOB SUMMARY: The post holder will be responsible for the development and implementation of the day-to-day clinical leadership of the Assertive Outreach Team which provides care and treatment to Service Users across all five Primary Care Networks within North East Lincolnshire.
You will be expected to lead on the assertive engagement pathway for the team and for the wider community mental health services.
You will be expected to advocate for and support Service Users who struggle to engage with mainstream services and support other team leads to ensure other services understand the needs of this service user group.
You will offer clinical leadership, ensuring the maintenance and promotion of high standards of person-centred care for service users as well as assisting with the development of staff within the team.
You will be expected to maintain, monitor, and facilitate the delivery of a high-quality Mental Health Service in the most efficient and effective manner within existing constraints.
You will be expected to provide strong professional/managerial and clinical leadership to the Multi-disciplinary Team.
You will be expected to provide strong leadership in respect of developing excellence in practice, ensuring that robust practice and personal development initiatives are available.
You will be expected to ensure national standards and local guidelines, policies, and care pathways relating to care and treatment are effectively delivered for the promotion and delivery of quality services.

Main Duties / Responsibilities:
Clinical:
To work across North East Lincolnshire providing mental health advice and leadership based on an agreed operational policy and evidence-based practice.
To assist in developing and evaluating models of care to enable effective communication and to facilitate best practice taking into account available resources.
Participate in multi-disciplinary team meetings, meetings associated with the Mental Capacity Act/Mental Health Act/Continuing Healthcare and team meetings.
To monitor, evaluate and ensure maintenance of the highest achievable standards of care.
Coordinate transfer or referral of service users to other teams or services where required.
Prioritise, plan, and coordinate own workload and activities in line with the changing needs of the team and service. This includes planning and monitoring client medication needs, making appointments for clients, conducting assessments, managing emergencies and enquiries, responding to crisis work within individual caseload, ensuring patient records are up to date and supporting colleagues on a daily basis.
Have the ability to work independently and to be able to resolve complex professional problems.
Crisis response to other staff's service users in the event of key workers' absence, and delegation from manager.
To participate in the implementation of an effective clinical supervision structure across the service.
Assess skill mix and identify areas of development within the Team.
Participate in the continued development of a comprehensive Risk Assessment and Management strategy for the service.
Provide professional support to staff by devising and implementing a framework for critical incident analysis and de-brief.
To provide and set up appropriate clinical training for staff across the Team.
To cover and represent the Senior Operations Manager in their absence as needed.
To be a positive team member, act as a role model for all staff grades, and to question concerns in a direct and responsible manner.
To ensure that all staff participate in a programme of PDR in line with Navigo policy.
To ensure that the actions and interventions of all staff are in accordance with relevant professional codes of conduct, the Mental Health Act (1983), the Mental Health Act Code of Practice, and the Mental Capacity Act (2005).
To work with and support the Senior Operational Manager with sound financial approaches to finances and budgets.
Person Specification
Experience
* Multi-disciplinary working with people with complex needs associated with Psychosis.
* Experience in Key working complex individuals who may be difficult to engage.
* Leading and supporting others with risk management.
* Experience of change management.
Additional Requirements
* Ability to work unsupervised and deal with crisis situations.
Knowledge
* Up to date knowledge of policy and procedure as it relates to service area with the ability to contribute to the development of local policies.
* Awareness of clinical governance and quality assurance issues as they relate to mental health.
* Broad knowledge base with commitment to evidence-based practice.
* Ability to demonstrate an understanding of the principles and values of clinical supervision.
* Knowledge and experience of research or audit.
Qualifications
* Registered Nurse/Occupational Therapist/Social Worker/AHP
* Willingness to undertake additional CPD as required by the Senior Operation Manager
