You would autonomously manage the assessment,planning, delivery and evaluation of patient care and utilise a whole systems approach when supporting admission avoidance for thisclient group. Main duties of the job As a specialist practitioner, you will deliver excellent patient care along a care pathway across organisational boundaries; your role willinvolve acting as a specialist resource and providing education to the wider multi-disciplinary team across the health economy.
