Job summary Hampshire & Isle of Wight Healthcare NHS Foundation Trust is looking for inspirational Bank nurses to deliver a high standard of quality care to patients within the community setting. Community Nurses work autonomously and as part of a team. Care is delivered in the patient's own home environment and designated treatment rooms. Our nurses are responsible for a manageable caseload, working across the city ensuring our patients are receiving the best quality care & thereby avoiding hospital admission. Hampshire & Isle of Wight's Community Nursing Service work with adults who need support for a multitude of health conditions. These include: Long term conditions management Medication administration Acute wounds & complex wound care Palliative & end of life care, including bereavement support. Facilitating discharge from hospital Continence care, including all types of catheter & bowel care PICC line management Main duties of the job Although a lone worker, you will be supported by an experienced & nurturing team, to allow you to develop your existing skills into becoming an autonomous practitioner in the community. You will be provided the training you require to maintain and develop your clinical skills. This will be achieved by attending appropriate training and working with Educators in Practice & other staff within the team.
