Job summary Coastal Medical Partnership is a group of GP surgeries serving 34,000 patients across 4 sites in Barton on Sea, New Milton & Lymington in Hampshire. An exciting opportunity has arisen for a part time experienced Practice Nurse, ideally with diabetes or lead nurse experience to work at our Barton site. We are looking for a candidate full of enthusiasm and ready for the challenge to join our nursing team. We are dedicated to patient-centred care and making a positive difference in our community. Main duties of the job To provide appropriate nursing care including health promotion and disease management to the patients of the practice. To document and evaluate need and delivery of care. The completion of health checks, health screening, immunisation and vaccination programmes. To lead on a designated area, involving call/recall and education/updates. Provision of health and lifestyle advice. Undertake immunisations, injections and vaccinations. Carry out screenings, e.g. lipids, blood pressure, cervical smears, etc. Assist doctors with procedures as necessary. Provision of long term condition clinics as appropriate (eg. CHD, Asthma, Hypertension, Diabetes etc) Provision of complex wound care Undertake ECG testing. Provision of travel advice. Venepuncture and the taking and preparing of pathology samples.
