To provide therapy to patients within their own home, including assessment and treatment for vulnerable patients with complex needs, patients in crisis, and those supported through discharge from acute hospitals or intermediate care bed-based units within the community.
To act as the patient's advocate throughout their rehabilitation programme, enabling patient empowerment through the process. Responsibilities include:
1. Communicating complex patient-related information effectively.
2. Ensuring collaborative working with multidisciplinary team members across health and social care sectors.
3. Using a range of verbal and non-verbal communication skills to progress rehabilitation and treatment programmes.
4. Supporting patients in self-management of their condition by ensuring understanding of their diagnosis.
5. Using communication skills of persuasion, motivation, empathy, tact, and explanation to gain informed consent from a wide variety of vulnerable patients.
6. Identifying barriers to effective communication, including cognitive impairment and anxiety, and developing strategies to overcome these barriers.
7. Addressing language and cultural diversity issues in communication.
8. Maintaining accurate and up-to-date documentation in line with legal requirements and communicating assessment and treatment results to appropriate disciplines.
9. Acting as an advisor on professional and clinical issues.
10. Communicating effectively with relatives and carers regarding patient care and progress with the consent of the patient involved.
