Position Overview
Coordinates and oversees all aspects of resident focused delivery of care. Provides direct supervision, leadership and education to other members of the health care team. Acts as a resource and advocate for residents, families and staff. Promotes the values, goals, and philosophy of the Home while ensuring a safe and secure environment for all residents.
Essential Duties
1. Development and modification of care plans in response to residents’ change in status.
2. Liaison with other health care personnel, agencies, and families.
3. Assist in the investigation, documentation, resolution, and reporting of complaints and incidents (i.e., suspected abuse).
4. Engage in meaningful interactions with residents during your entire shift.
5. Maintain good relationships with Medical/Support/Home staff, residents, families, and the general public.
6. Supervise all activities on the unit.
7. Monitor performance and complete performance evaluations for PSWs.
8. Direct, assign, and evaluate the work of PSWs.
9. May be in charge of the Home as required.
10. Utilize leadership skills to identify problems and engage in problem-solving on the unit.
11. Monitor that care delivered is consistent with the resident Bill of Rights.
12. Coordinate all quality improvement activities on the unit.
13. Understand the nature and meaning of quality indicators used by the Home and which indicators are publicly reported.
14. Participate in the improvement of indicators and achieve satisfactory results.
15. Actively listen to resident and family concerns and take appropriate action to resolve issues in accordance with the policies of the Home.
16. Transcribe and process physician's orders for medication or other treatment in accordance with CNO standards.
17. Order and ensure nursing supplies and equipment are properly and efficiently used.
18. Administer medications and treatments in compliance with the policies of the Pharmacy provider, the Home, and the Standard of the College of Nurses.
19. Document and report all errors and omissions.
20. Complete and ensure resident's clinical care assessments, care plans, and documentation are accurate and up to date in accordance with CNO standards, communicating changes to the interdisciplinary team, residents, and SDM/POAs as required.
21. Attend and participate in resident care conferences as required.
22. Maintain accurate and complete records of nursing observations and care in compliance with legislative requirements and the policies of the Home.
23. Assess changes in health status and report changes to the Substitute Decision Maker (SDM) and interdisciplinary team.
24. Complete assessments (RAI-MDS and other assessments) and develop and update the plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
25. Inspire others to be motivated and work with passion, empowering staff.
26. Any and all other tasks as assigned.
Qualifications
Current Certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse or a Registered Practical Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60.
Basic CPR
Experience in long-term care setting is an asset.
Ability to work with and relate to interdisciplinary team members.
Ability to work from the heart and make others smile.
Well-developed written and oral communication skills.
Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired.
Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, and must be able to provide three supervisory references.

Education
Preferred College or better.
Licenses & Certifications
Preferred Reg. Practical Nurse.
