Job summary We are looking for an enthusiastic and compassionate Social Prescriber to join our Wellbeing Services Team. As a Social Prescriber, you will support patients by connecting them to healthcare services, community groups, and statutory services that address the wider social determinants of health. This role is essential in helping patients improve their health and wellbeing through non-medical interventions. Main duties of the job Work closely with GPs, healthcare professionals, and local organisations to support patients in accessing community-based services and activities. Provide personalised, one-to-one support, helping patients take control of their health and wellbeing. Assist individuals in overcoming barriers such as social isolation, housing concerns, financial difficulties, and lifestyle challenges. Co-produce personalised wellbeing plans and facilitate engagement with relevant community groups and statutory services. Maintain and prioritise a caseload, ensuring timely and appropriate support for patients. Work collaboratively with the voluntary, community, and social enterprise (VCSE) sector to develop and strengthen referral pathways. Ensure all safeguarding policies and procedures are followed appropriately. Use data capture and case management systems to record interactions, progress, and outcomes. Actively promote social prescribing and its benefits to patients, professionals, and the wider community.
