We are seeking a dedicated and professional Referral Coordinator to join our Community Single Point of Access (SPOA) team on the Isle of Wight. In this role, you will conduct comprehensive patient assessments, provide expert advice, and direct patients to appropriate resources. You will serve as a key member of our inter-professional team, executing referral reviews and telephone triage, and collaborating with healthcare professionals to deliver high-quality services across a diverse range of community services.
As the initial point of contact for service users, you will provide a welcoming and supportive environment, guiding and supporting them throughout their treatment journey. You will advocate for service users, ensuring they are on the correct treatment pathway, and facilitate effective communication between staff and service users. This role requires a commitment to Health and Safety protocols, ongoing professional development, and may involve weekend and bank holiday work on a rotational basis. Main duties of the job
1. Conduct comprehensive patient assessments, provide expert advice, and direct patients to appropriate resources as a key member of the Community Single Point of Access (SPOA) team.

3. Collaborate with healthcare professionals to deliver high-quality services to clients across a diverse range of community services.
4. Engage in an integrated service model, delivering multifaceted interventions. Please note that this role may necessitate weekend and bank holiday work on a rotational basis in the future.
5. Uphold all Health and Safety protocols within the team to ensure a safe and secure working environment.
6. Contribute to the ongoing development of competencies and skills within the community setting.
7. Ensure regular participation in Continual Practice Development (CPD) activities to enhance the knowledge base of the referral coordinator.
8. Act as an advocate, adhering to the teams protocols, objectives, and procedures. Guide and support service users in navigating the correct patient pathway. Facilitate effective communication between the service user and all relevant aspects of community healthcare services.

Person Specification
Qualifications
* NVQ3 plus additional training to Diploma level or equivalent through short courses and relevant applied experience
* IT skills for keyboard, and skills with a range of Microsoft Office and other computer applications for writing, presentations, desk top publication, email and data collation
* Experience of any services referring into a Single Point of Access
* Health and Social Care foundation Degree
Experience
* Knowledge of nursing and / or physiotherapy / occupational therapy/ speech & language therapy / Community Rapid Response / MSK procedures and practices
* Significant experience of working in a relevant health or social care setting
* Knowledge and understanding of working with disabled and older people.
* Understanding of professional conduct
* Health/safety and risk awareness
* Knowledge of stroke rehabilitation
* Understanding of facilitating independence
