Take a role for care delivery under the direction of qualified nurse/deputy/unitmanager. Actively encourage patients to become involved in all aspects of their lifestyle.This includes the monitoring and updating person centred plans. Support patients to identify their own health needs and support themaccessing health professionals and facilities. Identify and provide practical and emotional support for patients to engage ina range of everyday tasks, which include supporting patients with theirpersonal hygiene needs, cooking, shopping, cleaning, managing finances,budgeting and all aspects of patient engagement. The post holder will support patients to access and participate in social,leisure, educational and occupational activities within the unit and the widercommunity. This will involve escorting patients away from the unit following afull risk assessment. Identify possible risk situations and report them to the manager and follow riskassessments. All staff will be trained to an agreed level of Safety Intervention Training.Once this training has been achieved, the staff member will be expected torespond to incidents that may include physical aggression which as a lastresort may result in restraint. To act as a key worker for patients. Perform and record clinical observations in line with Trust policy. To have knowledge or be prepared to learn about the CQC requirementswithin an inpatient setting.
