The post holder will be a key member of theAdult Speech and Language Therapy Team(ASLT) andCareHome Support Team(CHST). CHSTteam includes a Pharmacist, Registered General Nurses, Registered Mental Health Nurses, Palliative care specialist nurseand Therapists.BasedatUptonhouse inSloughyou will be a team player within theEastBerkshire locality.
The serviceoperatesMonday to Friday, 9-5. Thisis a17.5hourspart time band 4permanentrole.
• To provideDysphagiatrainingprogrammestocare homestaff
• Toauditimplementationof trainingincarehomesandadapttrainingpackagesasappropriate.
• To requestand collectdataonthe setobservationscarriedout in the carehomesaspartof the audit informationrequiredfor this service.
• To provide guidance to care homes onASLTreferralprocesses andsendrelevant materials.
• To create necessary resources to support care home staff to follow dysphagia and oral hygiene recommendations
• To promote the use of Dysphagia screening using triage screening assessment.
• To advise on processes tomonitorclient to support SLT assessments e.g. oral hygiene logs and food diaries.
