The Service The Community Rehab Service (CRS) is an integrated health and social care service designed to support adults with health and care needs to continue living in their own homes as independently as possible for as long as possible. We are ambitious in our plans to deliver innovative and effective services that put the customer at the centre of what we do, working in more flexible ways to understand how we can put the right support in the right place at the right time to help people live the lives they want to live. By doing this, our aim is to improve the quality of care provided, reducing demand for social care services, reducing acute admissions (and therefore increased demand for ASC services) and length of time spent in hospital, as well as reducing the dependency on long term care. What is the role This is an exciting opportunity for an Occupational Therapy Assistant Practitioner to work in CRS where you will assess and provide the appropriate interventions to Portsmouth residents in order to restore, maximise or prevent deterioration in their physical, psychological and social wellbeing. Working with the client and their carer(s), you will delivery flexible, holistic support plans, encouraging them to practice new skills to enable them to maintain independence. You will provide assessment and advice through a problem solving, strength-based approach and provide equipment and adaptations which meets peoples needs and maximizes independence. What you'll need to succeed You will need good literacy and numeracy (A - C in Maths and English) or equivalent qualification and experience of working in a care related field e.g., social care, health, therapeutic setting. With a thorough understanding of the principles of rehabilitation you will have experience of completing comprehensive assessments and the ability to formulate, modify and evaluate rehabilitation programmes. You will positively and clearly communicate to a wide range of people, in a wide range of formats which will include writing clear, concise reports/letters and using IT systems. With excellent organisational and customer service skills you will have the ability to look for and act on opportunities, including working with others, to improve services for clients, with regard to promoting independence and reducing risk. The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post. You will be required to be flexible to work occasionally outside of office hours with the ability to travel around the city.
