Job summary Are you interested in undertaking this exciting role of Macmillan Integrated Personalised Personalised Care Programme manager? The post is a fixed term post for 24 months on a full time basis working within Harrogate District Foundation Trust. The successful applicant must have excellent communication skills and work well as part of a small team. Previous experience of working with cancer patients or within a cancer setting is essential. The purpose of the role is to improve patient experience by leading on the development and delivery of integrated Personalised Care (a Community Model of Support) across Harrogate Place and the acute Trust so that care and support is received close to home where appropriate and reconnects patients and families with their local community embedding a sustainable integrated cancer pathway. Main duties of the job Scoping the current position related to personalised care interventions both within secondary care and across the community and primary care setting working with key stakeholders. o Leading on collaborative working initiatives across services including secondary care, primary care, the living well service, community beacons and the Macmillan Cancer Information Service Data sharing across VCFSE partners and wider, eg:- secondary and primaryhealth care partners. o Production of an inclusive education plan and provide training where appropriate. oUndertaking comprehensive Co-production exercises with patients and families / carers listening to their lived experience to understand the current state and what would make a difference.
