Are you a conscientious and adaptable GPhC Registered pharmacy technician looking for a unique and innovative role that supports service improvement?
The Health Board is investing in an exciting new opportunity for an enthusiastic and proactive pharmacy technician to join the Pharmacy and Discharge/Step Down Lounge teams at The Grange University Hospital.
The pharmacy technician will be working closely with the Discharge/Step Down Lounge team and will be responsible for:
* Undertaking transcription of medications ready for discharge, working closely with the ward-based pharmacy staff to facilitate timely discharge, therefore improving patient flow.
* Counselling patients on medications to reduce the incidence of re-admission due to poor compliance.
* Ensuring step-down patients transfer their medication with them to an eLGH / Community Hospital to avoid the duplication of supply and therefore cost avoidance through reduced wastage.
* Support governance for medicines management at the Discharge/Step Down Lounge.
As well as supporting Discharge/Step Down Lounge, this post will involve working with the wider pharmacy team and supporting the education and training of less experienced technicians, trainees and support staff (an excellent development opportunity).
Main duties of the job
To provide a medicines management service to acute wards and participate in dispensary sessions at GUH.
To work closely with the Senior Pharmacy Technicians and Pharmacists to assist in the daily running of the Medicines Management ward-based service.
To use the Pharmacy and other computer systems (e.g., CWS) to carry out all tasks in an accurate and timely manner, ensuring a safe and efficient Pharmacy service can be provided whilst delivering the highest quality patient care through personal actions and continuous improvement.
To supply medication suitably labelled for discharge, including checking suitability of patient's own medication.
To transcribe drug histories or reconciled drug charts to the 'Discharge notification' section on CWS on or during admission.
To accurately check the dispensing of other members of the department, following completion of ABUHB ACPT reaccreditation that meets the requirements of the National Education Framework.
To communicate accurate information to patients, carers, members of pharmacy and other healthcare professionals.
To comply with Health and Safety and COSHH legislation to ensure the safe handling of all pharmaceuticals.
