Job summary
We have an exciting opportunity for a community support worker to join the York and Selby Early Intervention in Psychosis team. This new post will focus on the delivery of physical health monitoring and support for people aged 14-65 experiencing a first episode of psychosis.
This post is not open to trainees therefore candidates must have the following qualifications to apply for the post - and copies of these will be required prior to interview:
Diploma in Health and Social Care level 3 or equivalentCare CertificateKey skills in Literacy and Numeracy level 2Successful completion of Trust's full HCA Framework within 6-month probationary period
Main duties of the job
Supported by a Nursing Associate and Clinical Nurse Specialist you will run your own physical health clinics in sites across York and Selby, including visits to patient's homes when required. Main duties will include blood taking and ECG monitoring, physical observations and offering advice and support around health living. You will need to be an accurate record keeper and be able to work collectively as part of a large multidisciplinary team.
