A sensational opportunity has arisen to join the well-established, innovative, and dynamic Adult Integrated Respiratory (AIR) Team that is part of the Frimley Adult Integrated Respiratory (FAIR) Service.
We are looking for an enthusiastic and knowledgeable B7 Clinical Nurse Specialist/ Specialist Practitioner who is committed to delivering excellent, evidence based respiratory care with a special interest in interstitial Lung Disease. You will be part of a senior team, leading and developing nurses, physiotherapists, nursing associates, associate practitioners, physio technicians and administrative support to deliver care for people with respiratory illness in the place of choice. A special interest and knowledge in ILD is essential.
The award-winning team is based at KEVII Hospital, but we also see patients in Frimley Hospital, in clinics and in their own homes. The focus is supporting people to manage their condition within the most appropriate setting. No two days are the same!
The job will require working in other designated locations across the ICB as well as the primary base. Flexibility is required across secondary care and community.
At present the role is predominantly Monday - Friday with the hours being 9-5 (although there might be some requirement to work 8-4 and weekends at times).
Main duties of the job
1. Assessing and reviewing patients who are on long term oxygen therapy within their usual place of residence or clinics.
2. Act as an expert resource on respiratory care to patients, healthcare professionals, and the general public.
3. Coordinate and care for patients with respiratory illness to ensure they receive the highest quality care.
4. Review patients at home, in clinic or hospital and support the Virtual Ward model to ensure the same or improved clinical outcomes.
5. Plan, deliver and evaluate patient care during their period of acute illness.
6. Use effective counselling/educational skills, including breaking bad news on diagnosis or prognosis of a chronic condition.
7. Contribute to palliative care of respiratory patients as they approach end of life.

Person Specification
Experience
* Substantial clinical experience (postgraduate) in the same or related specialty
* Some recent clinical experience in the acute or respiratory setting
* Previous experience in developing standards, guidelines and policies
* Experience in acting as a clinical supervisor and mentor to others
* Previous community nursing experience
* ABG interpretation
Qualifications
* Registered Nurse - Part 1,2,12
* Relevant specialist post basic program in respiratory care and management or relevant community nursing qualification
* First degree in nursing, or demonstrable experience at this level
* Teaching/assessment programme
Skills and knowledge
* Advanced clinical assessment skills
* Able to work autonomously and within a team.
* Previous experience of formal / informal teaching of patients & staff
Special requirements
* Full driving license and access to a car for use to travel across sites and perform domiciliary visits.
* Willing and able to work weekends and unsocial hours.
