The successful candidates will support our innovative and visionary model of care with practitioners working across various care settings, supporting and managing complex patient care needs, early intervention to prevent unnecessary admissions to hospital and interventions to prevent unnecessary prolonged hospitals admissions. Main duties of the job As part of the role, the successful candidate will have knowledge, understanding and skills in clinical assessment, ability to work as part of the multi-disciplinary team, discharge-planning process and ability to work autonomously. This is an excellent opportunity for a senior nurse who has experience in managing complex patient care, excellent communication skills, and strong leadership. The role requires the successful candidate to demonstrate autonomous decision-making and ensure that clinical care is delivered safely and effectively. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. Person specification Education / Qualifications Essential criteria BSc / Grad Diploma in relevant discipline MSc in advanced clinical practice or working towards Relevant Teaching / assessment programme Phlebotomy Cannulation ILS ECG Taking Experience Essential criteria Knowledge of key professional issues, and NMC guidelines and codes, relating to professional practice Knowledge of current national guidance and its application in the work place Evidence of ability to work collaboratively Substantial experience Registration experience at Band 5 Previous experience in developing standards, guidelines and policies Experienced in audit and evaluation Experienced in acting as a clinical supervisor and mentor to others.
