Nottingham City Council is seeking to recruit a Health and Social Care Officer to triage, coordinate referrals, and provide information and advice to citizens and professionals in the first point of contact team in the Adult's services. You will play a key role in promoting the independence and well-being of the citizens referred by providing advice, support, and information. This helps citizens navigate through to the right services based on the information you have gathered in this triage process.
The role is a temporary, full-time position for an initial period of 6 months with a view to this being extended on a rolling basis and is offering a pay rate of £18.20 per hour. The role will be hybrid working with 2 or 3 days per week where you are able to work from home.
The ideal candidate will have:
1. Enhanced DBS check
2. Experience of working within Health and Social Care
3. Full UK driving licence
4. Ability to work on own initiative and within a team
