Salary: Up to £39,496 plus shift enhancements (Depending on Experience)

Supportive Transition Service
St Andrew’s community services offer innovative “own front door” single occupancy, bespoke properties, registered under St Andrew’s Healthcare. Each Supportive Transition Service (STS) provides an individualised person-centred approach for complex individuals who do not have a route out of hospital.
A role within STS represents a real opportunity to support individuals through a transition period from hospital. Treatment programmes involve significant periods of time outside and are focused on supporting community reintegration.
About you
You will have in-depth experience working with patients in a mental health service. Being a strong team player, you will build and maintain effective relationships across clinical teams. You will have good interpersonal skills and the ability to develop rapport with patients.
You will also be highly resilient, able to work under pressure, and possess good problem-solving skills. An active NMC registration is required.
Some of the main duties of a Senior Staff Nurse are to:
1. Plan, organise and deploy staff according to competencies and workload to meet patient needs.
2. Develop, implement, review and promote effective outcome-focused care plans to meet individual preferences and needs, attending and contributing to ward rounds.
3. Effectively communicate with all MDT members, managing care environments and reducing the use of restrictive practices.
4. Build and monitor ongoing relationships with patients, their families, and carers to encourage trust while listening to and interpreting their needs and concerns.
5. Ensure all patient records, risk records, and interventions are up to date and accurate.
6. Use reflection and supervision to learn from events and generate opportunities to improve patient outcomes.
7. Receive and provide clinical supervision.
8. Support the division to deliver specific outcome measures.
9. Complete required ward audits in line with the Clinical Governance strategy.
