Job summary Operating within the framework of the Hospital Discharge Services guidance (Aug2020), the post holder will work in partnership with colleagues in the acute sector, socialservices, continuing care and other community health services to ensure thatappropriate patients from the acute setting are assessed and transferred safely toappropriate community based services in a timely and seamless manner. Forappropriate pathways there is an aim to support transfer from hospital to communitywithin 2 hours or on the same day.To be responsible for ensuring smooth daily operations at the acute hospital bases andto act as a key point of contact and subject matter expert for community services tosupport with complex discharge queries. To lead daily huddles as required. Main duties of the job Make judgements involving highly complex facts requiring interpretation and analysis. To prevent avoidable admission to acute care by undertaking autonomous, holistic clinical, functional and risk assessments and liaising with community practitioners and A&E staff to initiate the provision of appropriate services in the community To prevent premature admission to long-term care by creative problem solving to find appropriate alternatives, using highly developed skills and knowledge of assessment andcommunication. To support and enable the process of safe discharge from secondary to Community care and assist in bridging the gap from hospital to home. To proactively promote a seamless and multidisciplinary approach to health and social careprovision.
