Job summary
An opportunity has arisen for a highly motivated and enthusiastic Band 6 nurse to join the Pancreas Transplant Nurse specialist Team at the Oxford Transplant Centre. You will be based in Oxford in an outpatient setting, the role involves covering peripheral outreach clinics so a full current driving license is essential. You will be working as part of a MDT and, after appropriate training, an autonomous practitioner.

Excellent communication, problem solving and time management skills are a necessity.
The post holder could have a renal nursing background or experience of caring for people with long term health needs.
Within three months of starting this post, and following a supportive period of training, you will join the recipient coordinator on call rota, providing a 24hr service, including weekends and bank holidays, coordinating all transplant offers for Oxford recipients.

Job Summary:
The Pancreas Transplant Nurse Specialist role is to assist in the coordination and management of patients within the Oxford Transplant Centre that are undergoing assessment and work up for a Pancreas transplant. The role involves conducting patient consultations addressing clinical, psychological and social needs under the supervision of the Lead Pancreas transplant nurse with clinician support. This may include clinics during the week and occasionally at weekends in outreach and referring centres as well as within OUH. The post holder also works within the wider Transplant Specialist Nurse team providing on call transplant recipient coordination 8am-8pm and at weekends (occasionally 24 hours)
Specialist Nurses will:
8. Work alongside the lead nurse in accepting patient referrals to agreed criteria and be identified as the key worker for an agreed patient/client caseload.

14. Contribute to the on-going development of the designated service. Participate in the Recipient Transplant Co-ordinator on-call rota following a three month period of orientation.
Main Tasks and Responsibilities
Clinical
15. Work collaboratively and in partnership with other health care professionals, offering advice to colleagues.
16. Attend transplant clinics as part of multi-disciplinary team, providing clinical review assessing health and psychosocial needs of individual patients, and provide care to meet these needs through a process of shared decision making. Accept referrals to agreed criteria, and work in partnership with other professionals to deliver co-ordinated programmes of care effectively and referring patients to other practitioners as required.
17. Deliver expert nursing care to patients in agreed practice settings, working as part of nursing/multidisciplinary teams.
18. Communicate with patients in ways that empower them to make informed choices about their health and health care.
19. Act as advocate for individual patients and their client group when required.
20. Provide appropriate education to patients and their families to promote health and encourage self-care.
21. Use communication skills to impart information to patients and carers, and provide them with advice and emotional support in hospital / at home as appropriate.
22. Maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting and contribute to clinical activity/data collection as required.
23. Participate in the Recipient on call rota providing day time cover for a 24/7 service for the unit in processing organ offers. Autonomous working and advanced decision making skills are required.
Management
24. Be involved in developing standards of care, practice guidelines and where appropriate Care Pathways for the patient/client group.
25. Contribute to the development and delivery of nurse-led initiatives for the benefit of the identified client group, in conjunction with medical and nursing/AHP colleagues.
26. Demonstrate and share clinical practice developments to provide evidence-based care, and contribute to service improvement in accordance with local policy initiatives.
27. Contribute appropriately to clinical governance activities that relate to own area of practice and patient/client group.
28. Support clinical and management teams in the delivery of Trust and Directorate objectives.
29. Deputise in the absence of the Lead Pancreas transplant nurse as required.
30. To act as a resource to medical / surgical multidisciplinary team members.
31. To demonstrate clinical expertise and engage in collaborative practice with consultants and other members of the medical team.
32. Establish links and develop a network with other transplant units in order to disseminate knowledge, research and experience to advance the standard of care and education patients receive.
Education/Training:
33. Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through self-education and attendance at relevant courses of study

35. Contribute to the development of specialist nursing knowledge and skills to enhance and extend the practice of other nurses and health care professionals within the service

52. Assist in the auditing of service provision and prioritise areas for development, using national benchmarks.
53. Provide statistical and / or other evidence to demonstrate efficient and effective use of resources in providing high quality service.
54. Disseminate audit and research findings among colleagues, and where appropriate, at wider local, regional, national and international level.
