An exciting opportunity has arisen for us to recruit an experienced and motivated renal nurse to join our team as a Renal home therapies manager. We are looking for an enthusiastic and motivated nurse who has a broad renal experience with significant skills in Peritoneal dialysis and competence in Haemodialysis. The post holder will demonstrate a sound understanding of renal nursing.
The successful applicant will demonstrate excellent communication and leadership skills. You will have outstanding organisational skills be able to work autonomously as well as manage the wider Home Therapies Team.
You will have outstanding organisational skills be able to work autonomously as well as being part of the wider Home Therapies Team. This is an excellent opportunity for career progression. You will manage the Home Therapies service which includes Peritoneal Dialysis, and Home Haemodialysis.
You should be a confident educator for both patients and staff and be able to have a flexible approach in meeting the needs of patients.
You will work collaboratively with the wider renal multi-disciplinary team and support the renal ward with staff training and clinical expertise.
The role includes travelling to visit patients in their homes as part of their care and follow up. Access to a car and the possession of a UK driving license is essential to this post.
• To be responsible for the operational and clinical management of the Renal Home Dialysis Unit
• To be responsible in ensuring the continuity of patient care between hospital, home and within the department.
• To provide excellent nursing care, use specialist knowledge to teach & educate patients, carers, undergoing Peritoneal Dialysis and Haemodialysis at home.
• To work collaboratively to lead and coordinate the development of home dialysis unit making best use of available resources.
• To provide effective and robust training programmes for patients/carers/staff
• To provide an effective and responsive service to patients and carers within the trust and in their homes.
• To provide practice development support to renal nurses.
• Participate in the procurement of dialysis consumables and the home delivery services as appropriate.
