Invicta Health has an exciting opportunity for a Practice Nurse to join the team on a full time basis at East Cliff Practice, Ramsgate.

Main duties of the job
Asa Band 6 Practice Nurse you will work to provide and maintain a high standardof nursing care for patients including treatment, preventative care, screening, andpatient education. The role will involve undertaking a range of nursingassessments safely and effectively and providing appropriate care and/ortreatment to patients in conjunction with GPs, Clinicians and the widernursing team. You will work to policiesand protocols and strive to improve standards of care to the practices patientpopulation. There may on occasion be a requirement to undertake home visits.
As an experienced Practice Nurse, you will provideleadership to the practice team in relation to a defined area of practice andsupport the lead nurse in the day to day management of the Practice Nurse team.

Job description
Job responsibilities
Clinical
11. Assist inand perform routine tasks related to patient care as directed by Lead Nurse andGPs. Advising and escalating any potential problems, issues or errors in atimely manner.
12. Undertake andoversee chronic disease management or specialist GP services includingmonitoring progress and response to treatment, identifying where changes tomedication may be necessary and seeking senior support to implement, processingreferrals where required, working to reduce unnecessary hospital admissions,and promoting the preventative aspects of nursing practice. Chronic diseases,specifically Diabetes but are not limited to:
o Asthma
o AnticoagulationClinic
o COPD
Once trained and competent, work within your scope of practice to undertake womens health management including but not limited to;
oCervical Cytology
oContraception includingreviews, depo injection and pill checks
13. Work to recognise changes in a patients condition and take appropriateaction.
14. Undertake wound and ulcer care including suture/clip removal, dressings,and assessment to include doppler studies.
15. Implement, monitor, and evaluate a plan of care for wound and ulcermanagement to be followed by Healthcare Assistants, Nursing Associates andRegistered Nurses undergoing training in this area of practice.
16. Undertake diagnostics tests including but not limited to:
o Urinalysis.
o Taking swab samples.
o Request pathology tests, for example urine culture.
o ECG and BP testing including 24hr ambulatory blood pressure monitoring.
o Pulse rate and rhythm.
o Venepuncture
o Blood Glucose monitoring.
o Weight management including BMI.
o Height measurement.
o Peak flow readings.
o Spirometry (where ARTP registered)
17. Administer adult and childimmunisations, vaccinations and injections including travel vaccines, under arecognised legal framework.
18. Ensure safe storage,rotation and disposal of vaccines and drugs.
19. Chaperone patients to theirhealthcare appointments when requested.
20. Assist with minor surgeryand invasive procedures when required, this includes but is not limited to:
o Joint injections
o Minor surgical procedures
21. Ensure appropriate healthpromotion literature is available.
22. Prepare and maintainenvironments and equipment before, during and after patient care in line withinfection prevention and control guidance.
23. Undertake housekeepingduties including, general tidiness and cleanliness of equipment, nursing roomsand treatment rooms ensuring room and equipment logs are kept up to date andavailable for auditing upon request.
24. Attend and participate inany MDT and practice meetings when required.
25. Ensure practice fridges arechecked daily, temperature recorded, cold chain maintained and reporting anyissues where relevant.
26. Keep up to date with anychanges or advances within the Nursing profession, completing annual competencyreviews and necessary training.
27. Communicate effectively,sensitively, and compassionately with colleagues, patients, carers recognisingneeds for alternative methods of communication.
28. Liaise and communicate withmulti-agency partners and third-party organisations as appropriate, adhering toorganisational and national information sharing guidance at all times.
Administration
29. Accurately input, update andextract information onto/from the Practice clinical system when required.
30. Record patient informationclearly and concisely, adhering to national and local guidance.
31. Liaise with and feedbackeffectively to the nursing team and other members of the multi-disciplinaryteam (MDT).
Infection Control
32. Support the Infection,Prevention and Control lead through the following measures:
33. Follow procedures aimed atreducing the risk of passing on the organisms that can cause infections.
34. Demonstrate a duty of carein following best practice in adherence to guidelines which is a fundamentalrequirement in underpinning the management of Infection, Prevention andControl.
35. Apply infection controlmeasures within the practice according to local and national guidelines. Thisincludes:
o Dealing with blood and body fluid spillages
o Decontamination of instruments and clinical equipment
o Reporting and treatment of sharps injuries
o Segregation and disposal of waste materials
o The collection and handling of laboratory specimens
o Universal hand hygiene precautions
36. Monitor and change sharpsbins within the practice when requested or identified.
37. Assist patients andcolleagues in adopting recommended infection control measures and health andsafety measures.
38. Effective cleaning andsterilisation of clinical equipment and working surfaces, preparingtreatment/consulting rooms prior to use.
39. Support the Lead Nurse inthe completion of relevant Infection, Prevention and Control audits.
Professional Registration
40. Work inaccordance with Nursing and Midwifery Council Code of Professional Conductensuring NMC registration is valid.
41. Ensure youcomply with requirements for revalidation and maintain the knowledge and skillsyou need for safe practice.
42. Continueprofessional development (CPD) to supplement existing knowledge to aid furtherautonomy within the role.
43. Disseminateupdates in current evidence to all in practice, particularly new learning fromstudy.
44. Recognise and work within the limits of your competence and scope ofpractice.
45. Work with the wider multi-disciplinary team to deliver services in linewith current best evidence, national and local guidelines.
46. Contribute to the development of policies, protocols and guidelines acrossthe organisation, ensuring compliance.

Miscellaneous
49. Offer and provide clinical supervision to other members of the practicenursing team.

52. Actively participate in educational programmes for whole primary healthcare team, including medical and nursing students and GP registrar, in relationto areas of clinical and practice nursing expertise.
