Job summary We are looking for 2 Band 6 Nurses to help us set up and deliver Intensive Community Engagement within South Shropshire Community Team. Intensive Community Engage is a Pilot project for 2 years within South Shropshire Mental Health Service looking at delivering person centred holistic care for adults with complex severe and enduring mental health needs that are often hard to reach and find it difficult to engage with traditional community service models. The post holders will need to demonstrate the ability to be flexible and creative working outside of traditional models of service provision. The team is part of the South Shropshire Community Team and will offer the flexibility of a step up/step down approach for clients with Psychosis that as a result of their illness may be struggling to engage with their usual plan of care and require more intensive support to help them through this challenging time, helping to prevent the need for hospital admission/crisis care, reduced length of stays in hospital if for safety reasons for the person or others hospital care was needed and robust discharge planning. The post holder will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the team and other areas of the Trust where identified as appropriate. Main duties of the job Working within a Multidisciplinary team to support hard to reach clients with complex mental health needs who may find it difficult to engage with traditional community service models. Adopt person centred approached to support people towards their individual recovery goals High Standards of communication skills including leading on daily huddles to aid team communication and also support the team Duty system giving advise and support to people and their families at times of distress.
