In partnership with DMD Care UK and Joining Jack, LTH are looking to recruit a highly motivated and enthusiastic individual(s) to project manage the development of National Standards of care and targeted toolkits for young people with a diagnosis of Duchenne Muscular Dystrophy (DMD) transitioning from children to adult services. This post is funded by Joining Jack and hosted by LTHT. The contract of employment and line management will be with LTHT.
The post-holder can be based in Leeds; however, applicants are welcome from across the UK provided sufficient time can be spent with the project lead in Leeds. This is a full-time post for a fixed period of 24 months.
Applications are welcome from a wide sector of individuals including those from a clinical, social or educational, or management background.
Main duties of the job
The successful applicant will work alongside the DMD Care UK transition working group as Project Manager (PM) to understand the needs of those dealing with the challenges of transition of care and becoming an adult. The PM will be integral to the development of specific guidance and pathways for care delivery that can begin to address the gaps/shortcomings in current transition services. The aim is to develop a robust young-adult DMD transition care pathway that addresses the specific clinical and psychosocial needs of this group, as well as identifying feasible, practical ways to improve transition in different health care settings.
Job responsibilities
The post holder will support the delivery of the DMD Transition Project. This is a national initiative bringing together patients, healthcare professionals and other stakeholders to ensure that every person with DMD has access to the best possible care. Active engagement with adolescents and adults living with DMD and their carers is critical to this project, and patient and public involvement (PPI) is embedded at every stage of the project. The creation of specific toolkits/leaflets/webinars or other resources relevant to transition of care and becoming an adult living with DMD is a key part of the project providing accessible information to better empower patients and improve care. Ultimately the aim is to support individual planning and care delivery that addresses the holistic needs of young adults with DMD at transition, to optimise well-being and quality of life.
The post-holder will work with all stakeholders involved in delivering clinical and psychosocial services to young people and their families. This will include frontline staff, clinical professionals, psychosocial, educational services and the charity sector. This is a national project and will require flexibility to travel across the UK.
The post holder will be working with the Transition working group and its chair to develop ideas, drive and monitor progress, and provide the additional support and resources needed to deliver their aims and objectives.

* Professional or degree level qualification or equivalent experience
* ECDL
Experience
* Has managed complex projects that have an impact throughout an organisation.
* Has successfully planned and managed projects in complex clinical environments which required clinical & non-clinical staff to modify their way of working.
* Can provide evidence of highly developed specialist knowledge in all aspects of application implementation and change management.
* This knowledge to have been attained by working in the NHS or within a large organisation.
* Experience in undertaking surveys and audits monitoring patient and staff satisfaction in relation to service change and ensuring that action plans are developed and monitored.
* Has a thorough appreciation of the opportunities that new technology creates.
* Evidence of continued professional development.
* Professional experience with DMD or the lived experience of a neuromuscular condition as a carer or young person.
* Has significant experience in the NHS or experience of managing projects in a structured environment ideally in accordance with the NHS standard for Project Management (Prince 2).
* Has experience of projects that span multiple organisations.
* Has experience of working with clinical teams.
* Knowledge of the NHS Plan and Modernisation agenda.
* A good understanding of business process re-engineering principles.
