Job Purpose:
Support Workers assist in all the core tasks within the remit of the support to provide for the physical, health, emotional and social needs of the people supported, and in assisting them to fulfil the aims of their individual care plan. It is expected that Project Workers operate at all times within Believe and Achieve Support Policies and Procedures and the legislative framework pertaining to young people and service users.
Nature of the Post:
Main duties will be carried out working shifts allocated by the Manager or delegated person, which will include weekends, evenings and sleep in duties and support Workers will be supported by suitably experienced staff. A number of flexible hours may be spent attending meetings, staff meetings, supervision, annual appraisals, training and reviews. In undertaking activities with the young people and service users you will be required to plan the journey with them as part of independent living and walk, assist on public transport.
The objectives are to successfully complete the Group’s induction and to fulfil the project worker accountabilities to a standard complying with the standards set out by Ofsted and Children and Young People Leaving Care. The nature of the work at Believe and Achieve Support is varied and in many instances unpredictable. All staff are therefore expected to work in a flexible way and tasks that have not been covered in the job description have to be undertaken.
Core tasks include some record keeping in accordance with legislation and regulation. At all time, the post holder is expected to conduct themselves professionally, to work within the current legislative framework and adhere to the Policies and Procedures.
Key Objectives:
To provide a caring, safe, secure and accepting environment that meets the basic physical, health, social needs and rights of the young people and service user. This will be undertaken via positive and effective team working, to achieve high quality standards of care and support that are set and agreed by the manager on behalf of Believe and Achieve Support Ltd. This work will aim to respect and promote the needs of each client arising from their racial, cultural and religious background.
Key tasks will include undertaking a range of domestic duties, including cooking and cleaning, to a good standard, complying with health and safety policies and guidelines for young people, service users and staff.
To make assessments of need and co-ordinate the formulation of plans to enable these needs to be met. This will include developing an awareness of the need to liaise with other agencies and taking into account the wishes and feelings of the client, their parents and other persons with responsibility, in line with legislation pertaining to children and young people and vulnerable adults.
To assist in enabling and maximising educational opportunities and achievements for people supported at Believe and Achieve Support by assisting in supporting educational provision or other relevant groups/activities.
Under the guidance of experienced staff, to undertake direct work with both individuals and groups that is designed to meet the client’s needs as identified within the care plan and assisting them to reach their full potential. Supporting the client’s significant relationships and working with families will be a part of this task.
To display a good level of awareness and understanding of protection issues, assessing and recording risk. This would include referring any protection issues to a member of staff or manager and incorporating consistent boundary setting into everyday practice, to ensure effectiveness in the prevention and appropriate management of challenging behaviour.
To develop the skills to maintain accurate and up to date records of the work undertaken with the client’s.
To key-work a young person, ability to complete the assessment frame work plan, and monitor the outcomes for the young person. In conjunction with the plan you will also be required to complete monthly reports to the placing authority.
You will need to be able to attend meetings involving your key client confidently, record all information discussed and produce minutes of the meeting. You will also be required to give verbal up-dates at staff meetings on the progress made from your key-client.
To have completed or be actively working on a QCF Level 4 Children and Young People Family Practitioner in Residential
To liaise with Social workers and families confidently.
To undertake such other tasks as may be deemed appropriate to the post and the remit of Believe and Achieve Support Ltd.
