Main duties of the job Conducting home visits for frail, elderly, and housebound patients, ensuring timely assessments and treatment. Providing urgent and routine care to patients in their homes, including chronic disease management, end-of-life care, and post-hospital discharge reviews. Supporting the wider practice team with urgent care, minor illness management, and long-term condition reviews within the practice. Working closely with GPs, community teams, and social care services to ensure seamless patient care. MSc in Advanced Clinical Practice (or equivalent experience) Independent prescriber (V300 qualification). Experience Essential Experience in primary care, community nursing, or urgent care settings. Strong clinical assessment, diagnostic, and prescribing skill. Full UK driving licence and access to a vehicle for home visits.
