You are experienced in triaging and conducting strength-based assessments for individuals with disabilities and their carers, and proficient in recommending, utilizing, and prescribing equipment and adaptations. You handle a range of tasks efficiently, working independently within the service's standards. Your skills involve executing assessments compliant with the Care Act 2014 while providing clear, evidence-based information verbally and in writing. Committed to continuous professional development, you understand the council's vision and values, and manage conflict and complaints adeptly. A DBS check is essential, and a UK driving license is advantageous. What the Job Involves This role requires working within the Care Act 2014 framework and City Council policies to offer strength-based assessments that promote independent living and enhance quality of life for adults, families, and carers. Core responsibilities include assessing eligibility criteria, recommending and coordinating necessary equipment and adaptations, and educating individuals and carers on their usage. You'll collaborate with stakeholders for optimal service delivery within resources, manage workloads according to Adult Social Care priorities and Council policies, and maintain electronic social care records. Further duties involve developing Occupational Therapy Assistants, supervising students, and participating in ongoing training. The position prioritizes efficient resource management, explores funding and income opportunities, and ensures activities enhance social value. You’ll contribute to the Council Plan delivery, provide exceptional customer service, promote equal opportunities, and adhere to health and safety legislation.
