Inpatient Case Manager Registered Nurse

Monday-Friday: 8:00am-4:30pm, Rotating Saturday shift
40 hours/week
Job Description:
* Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.
* Directly support those in-hospital patients identified in need of comprehensive case management.
* Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.
* Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver.
* Functions in one of the following practice settings: Hospitals Only.
Minimum Qualifications:
* Registered Nurse License - Single State License - Virginia OR
* Registered Nurse License - Compact/Multi-State License
* Nursing Experience - 18 months
* RN-Associate's Degree OR
* RN-Bachelor's Degree OR
* RN-Diploma OR
* RN-Master's Degree
* Communication, Critical Thinking, Service Orientation
Preferred Qualifications:
* Previous inpatient nursing experience
* 1-year acute care case management experience
BLS required within 90 days of hire and maintained thereafter. Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
As an Inpatient Case Manager with Sentara, you get to work together in an intensely collaborative way with the patients, family, and members of the medical team to provide patients with the resources they need and help prevent them from readmitting into the hospital.
This position's primary duty is customizing what is best for the patients. Work directly with newly diagnosed patients and their family by providing fundamental information on caring for themselves, assisting with the needs of follow up care and appointments. That would include helping with things like home health, long term care placement, home medical equipment, homeless and community resources. You will find that teamwork is exceptional, with everyone working together to ensure the best care for our patients!
