Job summary MENTAL HEALTH SERVICES FOR OLDER PEOPLE (MHSOP) CENTRAL REFERRAL HUB/UNSCHEDULED CARE SERVICE Do you enjoy fast-paced, varied work? Based at the Neville Centre at Leicester General Hospital, and currently a Monday to Friday 09:00 to 17:00 service, the team of Band 6 RMNs, Band 3 HCSW, Consultant Psychiatrists, supported by a Band 7 Service Lead and our admin staff, covers the Leicester, Leicestershire, and Rutland area. Travel is minimized by the use of remote working where possible. Our team has a dual remit: Duty Nurse for shift role on rotational basis And to provide a timely response to address immediate risk. This major aspect of our service involves carrying out a thorough mental health assessment, and liaising with other health and social care professionals as required in order to meet the needs of our patients. The team works with patients with organic and functional illness, and our involvement is short-term. Each day is varied and at times challenging, but above all is extremely rewarding. You will need to possess sound clinical skills and the ability to formulate collaborative care plans. A high standard of written and verbal communication is essential. Being responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of service users according to changing healthcare needs and varying levels of complexity. The post holder will use advanced skills and expert knowledge to assess the mental health and social needs of a defined client group, instigating therapeutic care plans and treatments based on the best available evidence in order to improve health outcomes. Working within a multi-disciplinary team to ensure effective care plans and treatments are in place for complex services users. This will involve co-ordinating care and treatment activities with other professionals or external agencies, including detailed planning of treatment and discharge plans to ensure the best outcomes for patients/service users, being responsible for the planning, allocation and the delegation of duties and care priorities to the multi-disciplinary team.
