We currently have afantastic opportunityfor an experienced Mental Health Nurse to apply for Band 6 CPN post inCity WestCMHT. The CMHT is part of the secondary care services, providing assessments and treatment packages for a range of service users aged 18-65 years, with complex mental health needs.
There will be opportunities to develop and University Courses will be available, for example Non-Medical Prescribing.
The post holders will maintain a clinical case load, fulfilling all the roles and responsibilities of a Care Coordinator. They will work effectively in the multidisciplinary team to provide high quality mental health assessments, treatment programs and manage risk. This work will involve patients and their carers in the delivery of evidence based interventions/treatments, recovery focused work. You will need to demonstrate enthusiasm and commitment to high quality care as well as possessing good therapeutic and interpersonal skills. The post holder will be required to work autonomously as well as within the MDT and other settings.
Main duties of the job
City West CMHT provide holistic and person-centred mental health services for people aged 18 - 65
The CMHT is a well-established multi-disciplinary team, consisting of Consultant Psychiatrists, Nurses and Healthcare Support Workers, Occupational Therapists and Psychologists. The teams provide face-to-face assessments and care for service users referred via Primary Care, In-patients and Crisis Teams.
The successful candidate will be responsible for the delivery of effective healthcare within the team, by performing comprehensive assessments of patient nursing needs, plan, implement and evaluate care delivery according to changing health care needs, taking responsibility for the care needs of individual and groups of clients/service users.
The key skills required for these roles include the ability to demonstrate excellent on-going assessment, risk assessment and problem-solving skills and excellent communication skills.
The post holder will collect, collate and fully accurately report patient information, maintaining accurate complete and contemporaneous nursing records in line with NMC and Trust standards. Involving clients/service users and carers / relatives in the assessment, planning, delivery and evaluation of care.
