An exciting opportunity has arisen within the Community Mental Health Team (CMHT) Area 1& 2, Central Clinic for a Registered Mental Health Nurse (Band 6) to join our busy team. The team operates Monday to Friday, 9am to 5pm except Bank Holidays.
Main duties of the job
We are looking for a motivated individual who can work independently and as part of the Multi-Disciplinary team.Excellent communication skills are essential as well as having a positive attitude to mental health care whilst ensuring individual needs for privacy, dignity, respect and confidentiality are upheld at all times.
The role will consist of working as part of a multidisciplinary team in undertaking the role of the Care Coordinator in the delivery of a high quality community mental health service under the framework of the Care Programme Approach and the Mental Health Measure (2010) Wales whilst maintaining the Recovery ethos at the forefront of nursing care.
The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

Job responsibilities
To assess implement and evaluate agreed care and treatment plans for each individual on the caseload which has previously been agreed with the service users / carers. To work autonomously but under the indirect supervision of a Band 7 registered nurse / team manager and in conjunction with unregistered Band 3 healthcare assistants, other health colleagues and colleagues from the Local Authority in delivery of a supportive service for those people under the care of the CMHT.
There will also be an expectation of the successful candidate to work as part of our duty team, which will involve mental health assessments and providing a point of contact for existing service users in the absence of their care coordinator.
The CMHT also provide a variety of clinics, including depot, Clozapine, Lithium and physical health screening clinics, which the role will also be involved with in terms of providing support for substantive clinic nurses during times of leave.
Ability to travel daily between sites and patients homes in a timely manner to meet the requirements of the role/service is essential.

Qualifications and Knowledge
Essential
* Registered Mental Health Nurse
* Evidence of post registration study e.g. post graduate diploma or equivalent registration experience
* Ability to demonstrate clinical knowledge base and implications of current legislation i.e. Mental Health (Wales) Measure 2010, Mental Capacity Act, Deprivation of Liberty.
* Able to demonstrate a good knowledge of the current care and treatment for adults with Mental Health needs.
* An understanding of the Clinical Governance agenda.
Experience
