Some of the Key responsibilities include: To represent the team and patients at multi-disciplinary team meetings and case conferences in order to ensure the delivery of a co-ordinated multi-disciplinary service and to integrate therapy intervention into the treatment programme. This will include discussion of patient care, patient progress and involvement in discharge planning and support seamless transition into adult services when required. To give advice and training on intervention programmes, including advice to carers, other professionals and agencies. To raise any clinical concerns about patients with line manager/specialist physiotherapists in a timely manner and be willing to seek support from senior staff To formulate and deliver individual treatment programmes based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and highly specialised knowledge of intervention techniques To support the Team Leader, Clinical Lead and other Highly Specialist / Specialist therapists as required. To work with the patients, carers and colleagues to identify goals as part of intervention To assume delegated tasks as requested by line manager, including participation in working groups, policy development groups.
