Job Purpose: To work autonomously and alongside Nurses, Physiotherapists, Occupational therapists, and Assistant Practitioners as a multi-disciplinary team member, to undertake comprehensive assessment and intervention to clients within the pre-admission areas of Nottingham University Hospitals Queens Medical Centre Campus, in order to facilitate the clients' discharge or appropriate pathway identification.
Dimensions: To include, but not limited to, supporting the team, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values. You will be working in a variety of pre-admission areas supporting discharge planning for clients, carers, and families using appropriate pathways and system partners. You will be required to participate actively in City Cares training requirements, including in-service training and multi-agency training.
This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness. You will be required to work primarily with the Queens Medical Centre but also across different bases and community settings which could include clients' own home, residential or nursing homes, or hospital.
Key Responsibilities:
1. Clinical: To undertake individual treatment and interventions with clients under the supervision of qualified staff. To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner (AP) role (as per AP guidelines).
2. To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment.
3. To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user.
4. To assess patients fully in line with their competency framework. This includes obtaining full collateral histories, assessing all elements of wellbeing as outlined (including transfers, mobility, Mental Capacity), and determining appropriate plans from the information.
5. Demonstrate effective communication to provide instruction, advice, and intervention to service users and carers in line with the multi-disciplinary team guidelines (as per AP guidelines).
6. To assess, provide appropriate equipment and demonstrate use of a wide range of equipment to promote independence (as per AP guidelines).
7. To assess for and commission a range of adaptations in line with the multi-disciplinary team guidelines (as per AP guidelines).
8. To attend and contribute to team meetings and case discussions using effective communication.
9. To maintain comprehensive and accurate records in line with policies.
10. To undertake a range of nursing, physiotherapist, and occupational therapy competencies, as per the AP competency framework.
Organisational:
1. To participate in the development of good practice for the AP role within Nottingham CityCare Partnership.
2. To provide support to other Assistant Practitioners as appropriate.
3. To be responsible for own Continuing Professional Development, incorporating it into your work as necessary.
4. To use information technology on a daily basis and participate in identified data collection and statistics and to maintain clear, concise and timely records in line with the service policy.
5. To always have regard for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties.
6. To actively participate in the services training strategy, including in-service training, and multi-agency training.
7. To participate by attendance and presentation at staff meetings, tutorials, and training sessions.
8. To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations.
9. To actively promote equality and diversity within service delivery and people's individual rights.

Education and Development:
1. To ensure knowledge, skills, and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines).
2. To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy.
3. To keep an active up to date CPD portfolio.
4. To assist in the training and development of others (new members of staff, students, and visitors to the service).
5. To keep up to date in basic IT skills in line with the modernisation of data collection records.
6. To participate in appropriate in-service training and individual learning with specific and measurable outcomes.
